Absentee-By-Mail Voting for Primary
Who can vote by mail Any NC registered voter- No special circumstances or reasons are needed.
How to vote by mail
Request a ballot- Your date of birth and the following form of ID: NCDMV license number / ID card or last 4 digits of SSN
Option 1: Online request for a ballot. Keep checking back if the ballot isn't available. It must be available by March 28.
Option 2: By paper using a fillable PDF form (you fill it out online, print, sign, and send) or a print-only PDF form.
Complete a ballot- This requires several steps

- Mark you ballot in presence of notary public or 2 witnesses
- Seal your ballot
- Sign the back of envelope
- Witnesses/notary public sign the envelope
They sign and print their name; provide their full address.
Witness must be at least 18y
If you received assistance, the assistant may sign the envelope
- Mail your ballot to be received by 5pm election day (now May 17). But send it in as early as possible.
- Return ballot in person to Polk County Board of Elections (Womack Building, 40 Courthouse St, Columbus) or to one-stop early voting site (also Womack Building)
- Only the voter, near relative or legal guardian may mail or hand-deliver the Return Envelope.
